The initial consultation with your medical oncologist is a comprehensive evaluation of your history and medical records. It may last up to an hour or longer. During this visit, you will have the opportunity to have your questions answered. We strongly encourage loved ones, caregivers, and family members to attend with the patient at this meeting.
Please plan to arrive a minimum of 15 minutes prior to your scheduled appointment time.
Please bring the following:
- Completed copies of the Patient Registration, Past History, and Questionnaire forms.
- Medication list
- Up-to-date Insurance card
- License or photo ID
List of questions or concerns to discuss with your doctor.
